Forest Hills Pediatric Associates is a qualified “Medical Home” practice, which means it’s our job, and our privilege, to care for your child’s complete health and wellness, and to work with your family as a whole to ensure your child has healthy and successful growth and development.


It means parents and patients can expect our commitment to developing a long-term relationship that provides preventative care, sick care, and 24-hour on call services for emergencies.


Our medical home is dedicated to developing a partnership between the child, parent, and our care team—the first place you consider for all your medical needs.


We believe that every child should have a primary health care provider who knows the child's complete medical history, and is able to provide appropriate medical services based on your individual beliefs and values.

    • Tell your doctor about any changes in health and well being.

    • Prepare for, and keep, scheduled visits or reschedule visits in advance whenever possible.
    • Call your doctor first with any problems, unless it is a medical emergency.

    • Notify us when your child receives a vaccination at a pharmacy or elsewhere.
    • End every visit with a clear understanding of your doctor's expectations, treatment goals, and future plans.

    • Care for you or your child to the best of our abilities based on our understanding of current methods available.
    • Give you/parents/caregivers clear directions about medicines and other treatments.
    • Refer our patients to trusted experts, if needed.
    • End every visit with clear instructions about expectations, treatment goals, and future plans.
